Newspaper/ promoting items

Can anyone actually confirm how items get selected for the newspaper, and what happens when you promote an item. I see items refreshed even before the clock counts to 0. Just trying to understand how it really works.

The newspaper always change (all item change)at the last 2-3 second, So I always wait
