I'm a bit confused. I'm lvl 12 but what's the third island and when will I get there.

After you complete Level 12 you'll get to the third island.

Levels 1-6 are on the first island, levels 7-12 are on the second, and 13-18 are on the third. From the map there are 6 islands that I can see. After you get to the 3rd island you'll get your 3 gems and another goal to get to the 6th island for 3 more gems.
