You have to put a person behind it. Any of the people from the floor decorating tab will work! Hope that helps.

The second cash register is intended to be used with a second door. Without the second door I believe the bots will continue to use whichever register was placed first or whichever register is closest to the door.

This is correct. You can have a second register as a decoration only. Make sure you pull it out from a wall and place another person or animal behind it.

If you have two doors and your bakery is separated into two separate bakeries, then both registers will be used.
