Need to transfer my Storm8id to my Iphone5...HELP!!!!

I also have "Design your home" as well. I just got the new iphone5 and it transferred "Bakery Story" but it put me back to the beginning. Can anyone help me out? How do I pick up where I left off. I was level 96 in Bakery Story and 10 in Design your home.
