You can't. There is only the selection between a male and a female image.

Looking through my list of neighbors, two people have actual photos next to their name. It can be done. Just would like to know how.

use facebook connect. the pic only appears in the invites tab.

To change your avatar click on the face that shows your citizens' happiness, you will end up on your "Profile". Once there, scroll down to the avatar picture and click the "Change" button. You can choose between a boy or a girl, not very detailed but, thats how it is.
