I have done this myself recently and thought I would share the address to Email and the information needed to make the change.
Email TeamLava at support@teamlava.com asking for a name change and including the following:
The farmer name you have now, as in the name that appears on your profile.
The current experience level of your farm.
The Storm8 ID for your account.
The name you would like your farmer to have.
