Expand to 19x22

I see the option to expand to 19x22 for 40 gems. This size is not unlocked until level 50. If I purchase this, will it expand to this size even if I have not entered Level 50?

You can always buy the expansion for gems even if you are before the unlock level.
Once you buy with gems the option for expanding to that size at level 50 will disappear.

If a restaurant is at 13x16, can the owner spend 40 gems to jump to 19x22, or would they first have to buy the previous expansion levels first?

I just tried it, you can't skip the expansion levels. You have to go in the correct order. Too bad...
